Does my parent need a smartphone?

No. If they have one, they see a single big-type screen with two buttons. If they don't, reminders arrive as an automated phone call to their landline or flip phone, and you still get the missed-dose alerts either way.

They'd hate feeling watched. How does this not feel like surveillance?

They see a reminder, not a monitor. There's no camera, no location tracking, no activity feed about them. The only thing your family sees is whether a scheduled dose was confirmed. It's the same thing you'd learn by calling, without the daily are-you-checking-up-on-me call.

What if they just tap "taken" without taking it?

No app can watch a pill be swallowed, and anyone claiming otherwise is selling something.
